The generation of meaningful laboratory results start before the time of sample collection, when the patient is prepared for phlebotomy, and is completed once the result is delivered to the clinician.

In the laboratory this process is divided into 3 phases:
• Pre-analytical phase (patient preparation, sample collection, transport, registration and sample preparation)
• Analytical phase (analysis of the sample to produce a result)
• Post-analytical phase (creation of the final report that reaches the clinician)

A review of the literature suggests that +/- 68 % of laboratory errors occur in the pre-analytical phase. This implies that the person collecting the blood and completing the request form plays a crucial role in the quality of the results produced. A recent in house analysis at PathCare demonstrated errors or lack of important information on request forms in approximately 22% of forms received.
